The team also announced that infielder Asdrubal Cabrera underwent surgery to repair his fractured left forearm.
Sizemore left the Indians game on Sunday against Baltimore in the fifth inning after apparently suffering the injury sliding into second base in the third inning.
The three-time All-Star has struggled mightily for the second straight season, though last year inflammation in his left elbow and an unstable abdominal wall on his lower left side curbed his production.
He had surgery in the offseason to correct both injuries, but the 27-year-old has hit just .211 with 13 RBI and no home runs in 33 games this season.
